Vehicle year, make, model and trim earns its place because trim determines fitment and because staggered setups and low-profile fitments change the price bracket entirely. Current tire size earns its place, though the form should let the customer photograph the sidewall instead of transcribing 225/65R17 correctly, because a mistyped size produces a quote that has to be walked back. Mileage and driving pattern earn their place because they turn into a replacement forecast later. Whether the vehicle has driver-assist cameras or radar earns its place, because ADAS recalibration is the difference between a $100 to $200 alignment and a $300 to $700 one, and a customer who first hears about that at the counter feels ambushed rather than informed. Name and phone earn their place. Almost nothing else does. The urge to ask for a preferred brand should be resisted in most shops, since the answer is usually whatever the last shop put on, which tells you very little.

The Alignment Question Belongs Inside the Funnel, Not at the Counter

Attach rate is decided long before the car is on the rack.
